1. To login to the system, the user must provide valid accreditation no,
user name and password.

2. In the Welcome Page, menu is located in the left portion:
a) HCI Information
b) uPCM
c) Tools

ENLISMENT MODULE

1. Under the Enlistment Module, the user can search for currently enlisted
clients or non-enlisted member/dependent of PhilHealth.
2. The search is either by PhilHealth Identification Number (PIN) or by

Name (First and Last Name) and Birthday.

3. If there are no records found, the system will display message; “No
Record Found”. (see Figure 4)
4. If there are records found in the search, the user can proceed to the
Data Entry Sub Module by clicking the PIN of the Member/Dependent. (see
Figure 5)

5. In the Data Entry Sub Module the following details will be displayed:

a. Case No vi. Extension

b. Date of Enlistment vii. Contact No
c. Eligibility Status viii. Sex
d. Membership Category ix. Civil Status
e. Client Information f. Address of Client
i. PIN i. Province
ii. Type ii. Municipality
iii. Last Name iii. Barangay
iv. First Name iv. Zip Code
v. Middle Name g. List of Dependents

6. After enlisting the client, the system will prompt confirmation message
and it will redirect to the Enlistment Client Data Page under Enlistment
Module. (see Figure 7)

7. Click the icon to proceed to the Profiling Module. Or click the

Health Profiling Menu on the left portion of the screen to search for a
client you want to profile.

Under the Health Profiling Module, the user can view and edit individual
health profile including: Editing is only allowed if done within the same
year.

1. Under the Tools, the user can view his/her profile under the View
Profile. (see Figure 9)This will display the following:
a. Hospital Accreditation No
b. Name of Online User
c. Designation
d. Email Address
e. Date of Activation
2. Under the Tools, the user can reset his/her password under the Reset
Password.
3. To reset the password, provide the new password and re-type the new
password.
4. To logout in the system, click the Logout Button in the Upper Part of
the Screen. (see Figure 11)
