If P35.2 is the right answer, where it has been mentioned as congenital, i.e., by birth, during birth, since
birth, etc.?

It has been mentioned in the chapter-specific guidelines about code P36 regarding congenital bacterial
infection; however, there has not been any instructions for viral infection to decide “congenital” as a
default code if not mentioned.

Punch biopsy – 11104, 11105; however, these codes are not given in the options.

11440 is excision of a lesion. I know there is no excision, but I have to select between the second and
fourth option; however, the fourth option has a deleted code and also has modifier 59 (Distinct
Procedural service) which I think is not needed here as modifier 51 (multiple procedure) is already exist.
