I, IVY C. MACUL, Filipino, of legal age, married,

and resident of Mantahan, Maasin, Southern Leyte, Philippines, having been
duly sworn in accordance with law, hereby depose and state:

That I am currently unemployed as of the moment due to the

retrenchment due to pandemic;

That I am not gainfully employed due to the said retrenchment

in my workplace;

As such, I am executing this Affidavit for the purpose of

submitting this to support my petition for change of first name in our
Local Civil Registry in Surigao City and to attest to the truth of the
foregoing facts and for whatever legal purpose that this Affidavit may
serve.
