:the questionnaire contain 36 questions about

noise hazards: about the awareness of the permissible level -1

of noise, the presence of periodical measurement of noise at
work place , using of hearing protection and periodical hearing
.examination of workers

electrical hazard: awareness of electrical hazards, regular -2

. check of electrical equipment
fire hazard: attending training to use fire extinguisher and -3
emergency evacuation, presence and state of emergency exits
.and storage of flammable materials

ergonomics: presence of mechanical assists to help material -4

.handling tasks and presence of suitable furniture

lighting: presence of adequate lighting and state of -5

.emergency lighting

temperature: presence of suitable temperature and -6

ventilation, regular monitoring of humidity and temperature,
awareness of heat related illness and pre-employment
.examination

radiation: presence of caution symbol in radiation area, using -7

PPE , regular monitoring of radiation level and safety training
.for workers and female in child-bearing age
