PARENTAL CONSENT / WAIVER OF

LIABILITY
As the parent or guardian of ________________________, I hereby give my full consent and
approval for my child to participate as a volunteer for the Typhoon Rolly & Ulysses
Donation Drive arranged by the College Supreme Student Council, Medicine Student
Council, and Red Cross Youth Council of CGHC. The packing of relief for the drive is said to
happen on 27 November 2020 (Friday) at 8:00 AM. I understand that there are certain
risks to my child’s participation as we are still on the pandemic, and I am willing to assume
these risks on behalf of my child. Moreover, to giving my full consent for my child’s
participation in the donation drive, I do hereby waive, release and hold harmless the school
and its administration, SSC, MSC, and RCYC.
