SEQUENCE OF BASIC JOB STEPS POTENTIAL HAZARDS RECOMMENDED ACTION OR PROCEDURE
1. Hose connection between laybarge and Trip, slip and fall Ensure deck is completely free from slippery
supply vessel surface, protruding and obstructing objects
prior to hose connection.
Walkway obstructed Place caution sign/barricade tapes were hose
crosses walkway.
Align hose properly to maintain free access.
Man-overboard Use required/adequate PPE and ensure
personnel work vest has been properly donned
prior to commencement of task.
Use competent/qualified personnel.
Avoid short cuts, follow approved fuel transfer
method.
Wrong connection Use qualified/competent personnel.
When not lucid with the process/procedure,
stop the task and ask your supervisor/lead
person for clarification.

Ensure hose are lined completely clear from
the vessel’s propeller.
2. Pumping bunkers Poor communication Ensure proper communication at all times.

parties.
The watchman shall be provided with a
suitable communication equipment to be able
to communicate with the vessel.

Environmental pollution (spill) Inspect hose properly and replace all doubtful
hose length prior to connection.
Ensure proper communication between the
discharge point and the receiving end.
Ensure pollution prevention equipment in place
as per vessel’s SOPEP.
Mooring shall be maintained to prevent strain
been imposed on hose by vessel movement.
The vessel crew shall be readily available and
close at hand throughout.
Pumping shall be by instruction and it must be
followed accordingly.
Fire out break / explosion Stop all external hot work.
Place drip trays in place.
Locate fire hose and fire extinguisher handy for
immediate use.
No smoking condition shall be imposed within
transferring area.
Poor hygiene Hoses used for portable water must not be
used for transferring fuel.
Use required/adequate PPE
