Assessor in Shielded Metal Arc Welding (SMAW) NC I.

I am GLENN HEYRES ALBERT, 31 years of age, married and presently

residing at Lahusan, Fishing Village, Malita, Davao Occidental. I am a
holder of SMAW NC I with certificate no. 16112401033532 and NTTC Level I
certificate no. 161611241200119 valid until March 3, 2018 and Trainers
Methodology Certificate (TMC) no. 16112401000007 valid until January 24,
2021.

Attached herewith are the following requirements for your perusal, to wit:

1. Accomplished Application Form (TESDA-OP-CO-04-F16)

2. 1 piece 2” x 2” ID Picture
3. Certificate of Employment
4. National Certificate Level I - SMAW
5. Trainers Methodology Certificate (TMC)
6. National TVET Trainer Certificate (NTTC) Level I

Further, I am willing to undergo supervised assessment of at least ten (10)

candidates as per required.
