1. That, I have been known as EDITOR JOHN MWINUKA since my child hood.

2. That, sometimes my names appears as EDITOR JOHN MWINUKA as in my

National Identity Card No. 19820101141150000116 and sometimes as
EDITH MWINUKA GAMA as it appears in my Tax Identification Number (TIN)
120-069-063 whereas the name GAMA is the name of my Husband
(EPHRAIM GAMA); the name I assumed after marriage.

3. That, I declare to anyone whomsoever that the above names are mine and so
used interchangeably. And I would wish the names of EDITOR JOHN
MWINUKA to appear in my TIN 120-069-063.
4. That, this is to confirm to anybody whomsoever and I hereby swear that I the
