AFFIDAVIT OF LOSS
I, CINDY BALLADA ALIMONDO, of legal age, Filipino, and resident of JA-315,
Pico, Km. 4, La Trinidad, Benguet after having been duly sworn to in accordance with
law, do hereby dispose and state THAT:

1. My older, sister Cheryll Ballada Alimondo, was bonafide holder of College

Nursing Diploma duly issued by Easter College;

2. The original copy of said College Diploma was submitted by my sister,

Cheryll, to a one Agency in Canada sometime in 2016, when she was
applying for employment. Unfortunately, her application was denied.
Thereafter, she requested the agency to return it back to her the original copy
of her Diploma, but the agency failed to do so until today. In view of her
earnest efforts to retrieve the original copy, however, her efforts prove futile,
she therefore considered it loss and beyond recovery without negligence on
her part whatsoever;

3. My sister’s College Diploma has not been intentionally lost by her, but due to
the afore cited reason;

4. I am executing this Affidavit to plead and request Easter College to issue a

new College Diploma in lieu of the lost one. That, I hereby bind myself to fully
excuse Easter College for any damage or liability that it may incur for the
issuance of the requested Diploma;

5. Finally, I am executing this affidavit to attest to the truthfulness of the

foregoing facts and for all legal intents and purposes it may serve well.
