Behavior Think Sheet

This document is a reflection sheet for a student to fill out after breaking a classroom expectation. It prompts the student to identify their feelings, which expectation was broken, what choice they made and how it affected others. It also asks the student to identify what need wasn't met that caused the behavior, if they had a coping strategy, and what they need to do to correct the problem. The teacher signs off after discussing it with the student to resolve the issue and allow the student to rejoin the lesson.
