CRITICAL THINKING QUESTIONS (CTQ): Concepts on NCM 116 (GIT Alterations)

Mr. Smith, a 65 years old male patient was admitted to a tertiary hospital. Upon
ER admission, the physician’s impression was “to consider perforated duodenal ulcer”.
At 9:30 in the evening from the medical-surgical unit, his wife runs to the nursing
station and says that you need to help her husband, he is in terrible pain. You, as the
charge nurse on duty, you entered his room and have seen Mr. Smith curled up in a
knee-chest position on the bed. He is crying and complaining of excruciating abdominal
pain.

1. What pertinent additional data should you have to gather? (30 points!)
I may ask the patient or important others about his lifestyle as a charge nurse
before admission, such as food intake or activities like smoking and alcohol
consumption, ask the patient where else the pain radiates, whether he
experiences pain in the back region or in a shoulder or in any part of his body,
measure the pain of the patient, ask him to rate the pain from 1-10, Ask the
patient if he feels nauseous or wants to vomit or feels dizzy, ask the patient or
significant others if the patient takes any over-the-counter drug such as NSAIDs
in the long term, and eventually review the patient's health history.
2. What nursing actions would you aim to complete? Give at least five. (50 points!)
 Administer medicine according to the doctor's order. Track vital signs, note if
tachycardia and hypotension are present. To prevent alteration of gastric
mucosa such as acid generating capacity, such as carbonated beverages,
coffee and spicy foods, instruct patients about specific foods. Encourage
patients to eat daily meals to stop eating too much. Instruct patients or
significant others to report signs and symptoms of the most common and
most severe perforation of GI bleeding / hemorrhage.
