This refers to your request for SGS Philippines, Inc. to undertake analysis of the following various samples:

NOTES:
 Above quoted fees are subject to 12% VAT. If company is zero rated, please submit certificate of
exemption.
 Minimum invoice amount per transaction is Php 3,360.00, VAT-Inclusive.
 Total Fees may vary upon receipt of actual sample or after assessment by the laboratory.
 Test will be performed and subcontracted to *SGS China. Any custom clearance charge will be
shouldered by the client.
 **Courier cost can be subject to change depending on the actual costing requirement and
weight/volume of the actual samples. Balance on the courier cost upon receipt of the actual invoice
from the forwarding company will be billed to the client.

 All other costs related to this project not mentioned on this contract as well as cost that will be
incurred in possible clearing of the samples in China customs will be charged at cost to the client.

Samples Submission: Reckoning date for samples received after 11AM will be the next working day.
Samples are received between 8am-5pm, Monday-Friday.
Other days may be arranged for special projects.
Sample size: 2 pieces per sample, 150 cm 2 stainless steel or glass plate fully coated with the
sample. Fully wrap the plates in aluminum foil and enclose in a sealed and labeled PP
or PE bag.
Turn-around Time: 20-25 working days from sample receipt reckoning date.
Payment Terms: 50% down payment prior start of testing, 50% balance prior release of report
Validity of proposal: 30 days from the date of issue

Payments should be made on agreed payment terms. Unless a shorter period is established in the invoice, the client
shall promptly issue payment not later than 30 days from the relevant date in the invoice (“due date”), otherwise the
total cost will be subjected to a 1.5% additional charge per month from due date, until payment is actually received.
SGS shall be entitled to immediately, and without liability, either suspend or terminate provision of the service
including the release of any analysis report, in the event of failure by the client to settle obligations.
The conduct of the analysis shall be further governed by the SGS General Terms and Conditions attached herein as
Annex “A”.
