Sub: Request for extension of Safe Practise Period

I am an applicant to CNO with application number ( RN: /RPN: )

As stated in my advisory report for RPN I submitted my competency assessment supplement (CAS) in
August 2019 and received an eligibility for the CPNRE exam on 12/11/2019.

However my safe practise period of 3 years will be due in February 2020, for the extension of which I
have worked with Mathru Hospital (Multi speciality & trauma centre),Bangalore from 9/02/2019 to
9/25/2019. The details of my association with the aforementioned hospital will be directly sent to CNO
by my employer.

Also, the form for verification of nursing registration has been dispatched via registered mail to CNO by
KNMC (Kerala nurses and midwives council) on mm/dd/yyyy with tracking number xxxxx.
Enclosed along with this letter is my proof for travel (Airplane ticket & Boarding Pass) to India and back
to Canada. The purpose of this visit was to work at Mathru Hospital (Multi speciality & trauma
centre),Bangalore.

The earliest date that I can take the CPNRE exam is Jan-Feb 2020, since my safe practise period is due for
February 2020 I kindly request you to grant me a grace period and extend my safe practise period until
the results of the CPNRE exam for Jan-Feb 2020 is declared.
