Objective Data:
- Post-surgical wound on lower midline of the abdomen
- With clean, dry and intact wound dressing

Action:
- Assessed wound dressing
- Encouraged patient to wear loose-fitting, non-
constrictive clothing
- Evaluated patient’s skin practices and hygiene
- Advised patient to increase intake of food high in
protein
Response:
- The patient verbalized understanding of the preventive
measures that she must observe to avoid further
damage or injury on the surrounding skin and tissue
such as keeping the dressing clean, dry and intact and
cleaning the wound properly.
- Two days after the surgery the patient displayed timely
wound healing.
