AFFIDAVIT OF UNDERTAKING

That I, MISCHELLE S. PANGILINAN, married and of legal age, residing at

#64 Brgy. San Isidro, San Simon, Pampanga and after been duly sworn to
in accordance with law do hereby depose and state that:

1. I am the Head of Audit for SN SMN Meat Product Corp.

2. In Dec. 27, 2020, the company requested for renewal of NMIS

(National Meat Inspection Service) for its Cold Storage Facility, at the
regional Office, San Fernando, Pampanga;

3. In response, the following documents were required to have

certified true copies on current date;

PTO-Permit to Operate Air Pollution Sources Control Installation

DP - Discharge Permit

4. I am executing this affidavit of undertaking to confirm my

responsibility and liability on the required certified true copy.

5. Finally, I am executing this affidavit of undertaking to affirm the

truth of my foregoing statements and for whatever legal purpose it may
serve.
