Principles of Muscle Biopsy

Indications
1. Muscle weakness – static and progressive
2. Differentiate myogenic and neurogenic causes of weakness
3. Inflammatory myopathy and collagen vascular diseases
4. Metabolic diseases affecting muscles
5. Drug induced myopathy
6. To assess disease progress and prognosis

Selection of muscle
1. Moderately affected or muscle in which disease is evolving should be selected
2. Ideal site – Quadriceps, deltoid, biceps
3. Should be taken from muscle belly away from tendinous insertion
Procedure
1. Open
a. Skin incision under LA
b. Avoid infiltration of anesthetic into muscle
2. Needle
Precautions
1. Do not crush
2. Do not stretch
3. Do not immerse directly into saline
