C. MENSTRUAL HISTORY
Mrs. Hampac’s had her first menstruation when she was 12 years old, on the year 2009.
Her menstruation usually lasts 3 up to 7 days, and usually on the following day on her first
menstrual date every month, the flow is heavy. She uses sanitary pads during the day and night.
She can consume up to 15 pads of sanitary napkin monthly. She always experience menstrual
discomforts such as dysmenorrhea every first 2 days of her menstruation.

D. BREAST HEALTH

Mrs. Hampac did not experience any breast problems and discomforts.

E. CONTRACEPTIVE USE
Mrs. Hampac did not use any form of family planning method.

F. MEDICAL HISTORY
Mrs. Hampac has never been hospitalized for any medical complications. She does not
smoke nor drinks alcoholic beverages. She did not take any medications that is not prescribed by
a physician.
G. FAMILY HISTORY
Mrs. Hampac’s mother and father has hypertension. In 2017, her father was hospitalized
due to the increase in blood pressure level. On both sides of her parents, her relatives, has also
the same condition.

H. DOMESTIC VIOLENCE
Mrs. Hampac has never been beaten by her husband, and does not suffer to any
emotional trauma.
I. MATERNAL ATTITUDE AND REACTIONS TOWARDS PREGNANCY
Mrs. Hampac’s pregnancy is planned. During the first trimester, she is in the period of
confusion and denial but in the succeeding trimester, the 2 nd and 3rd, she is comfortable and
excited for the birth of their first baby. As part of their preparation, they had already bought
clothes for the baby, renews the setting of their house and prepares everything for welcoming
the baby as the new member of the family and as an individual to care for.
