AFFIDAVIT OF ADMISSION OF PATERNITY

I, ERWIN CABASAG AWIN, of legal age, and presently residing at Bulagao,

Tuao, Cagayan, after having been duly sworn to in accordance with law hereby depose
and say that:

1. I am the biological father of the child, DIONICIO MALANA, who was born on
May 8, 2002 in Bulagao, Tuao, Cagayan.

2. At the time of birth of said child, I was not yet married to his mother,
GERALDINE MALANA AWIN.

3. I hereby acknowledge and admit my paternity/filiation to the child, DIONICIO

MALANA.

4. I am executing this affidavit to attest the truth of the foregoing facts, and for
whatever legal purpose it may serve.
