This is to certify that Mr. Mark Joshua Gotauco, 24 y/o male from Paranaque
City consulted last January 22, 2021 for medical evaluation and clearance.
Patient presented no symptoms such fever, colds, cough, difficulty of breathing,
diarrhea nor impaired sense of smell and taste. By medical history, patient had
no symptoms and no probable exposure to Covid 19 positive patient for the past
14 days.
Clinical Impression: Essentially Normal at the time of consult

Remarks: no known direct and/or continuous exposure to Covid 19

patient
- No identified risk for Covid 19 infection

Disposition:
- Fit to work
- Observe recommended health standards and proper precautions
- for medical reevaluation once possible exposure to Covid 19 is noted

This certificate is issued upon patient’s request for work purposes and could not
be used for medico – legal purposes.
