Directions: Please rate the student based on the competencies listed below. Indicate the rate on the
space provided for using the following as guide:

PROCEDURE Rate

1. Assess client’s physical abilities, level of comfort, and activity intolerance.


2. Check doctors order.
3. Perform hand hygiene.
4. Identify the client. Explain the procedure to the client.
Sitting Position
5. Position client in an upright on the side of the examination table, the client can sit
on the edge of a chair or bed.
Supine Position
6. Client lying with legs together on the examination table. A small pillow may be
placed under the head or raise the head of the bed
Dorsal Recumbent Position
7. Client in a lying down position on the examination table or bed with the knees bent,
the legs separated, and the feet flat on the table or the bed.
Note. Abdomen should not be assessed when in dorsal position because the abdominal
muscles are contracted.
Sim’s Position
8. Client lies on the right or left with the lower arm placed behind the body and the
upper arm flexed at the shoulder and the elbow. The lower leg is flexed at a
sharper angle and pulled forward.
Standing Position
9. The client stands still in a normal, comfortable, and resting posture
Prone Position
10. The client lies down on the abdomen with the head to the side.
Knee-Chest Position
11. The client kneels on the examination table with the weight of the body supported
by the chest and the knees. A 90-degree angle should exist between the body and
the hips. The arms are placed above the head; with the head turned to one side
and a small pillow maybe used for provide comfort.
Lithotomy Position
12. The client lies on the back with the hips at the edge of the examination table and
the feet supported by stirrups.
13. Perform hand hygiene.
14. Document all observations.
