CASE ANALYSIS

You are a nursing student caring for a patient on isolation precautions.


The patient is male and is 72 years old. He is diagnosed with clostridium
difficile infection. He wears an adult brief due to incontinence of stool.
The patient has requested assistance with bathing and changing his
brief.

What transmission-based precaution will you implement in rendering care for


the patient? Discuss why. What PPEs are you gonna wear upon entering the
patient's room?

Transmission-Based Precautions to be implemented will be Contact Precautions


and can be paired with Standard precaution once needed in rendering care for
the patient. As spores from C. difficile bacteria are passed in feces and spread to
food, surfaces and objects. Wherein the said spores can continue to live in a room
for weeks or months. PPEs to wear upon entering the patient’s room will be
gloves and gown. Also, to note that one should perform appropriate hand hygiene
before entering and right after exiting the patient’s room. Additionally, to change
gloves and gowns and perform hand hygiene when moving from one patient to
the next, and before leaving the room.
Cleaning of the environment on a daily basis should be practiced, especially in
high-touch environments, horizontal surfaces, partitions, and the toilet, as well as
ensuring that the patient has clean clothes, dry incontinence supplies, and
practices hand hygiene in between treatment.
What health teachings are you going to give the patient and his family members
to stop the spread of infection?

The patient and his family members and possible visitors need to know that
wearing gowns and gloves are necessary when entering the room. They also need
to know the proper way to wear the gloves and gowns and the proper way to
remove the gloves and gowns before leaving the room. Appropriate handwashing
should be done in between them as well.
And, to teach the patient and family that they must be in a private room and that
being in the room as long as possible is necessary. To limit the transmission of
infections to other people, patients should only go to general places for testing
and treatments.

The patient is on isolation, how may this impact the patient? How are going to
intervene?
The majority of patients on contact precaution are still on isolation precaution.
Communicable diseases and multidrug-resistant pathogens infect them often.
Patients are often neglected by some, including healthcare professionals, leading
to feelings of sadness, fear, and frustration because they believe their needs are
not being met and because they are being treated as vile creatures.
It is essential to have optimal, holistic support and to keep track of a patient’s
physical, psychosocial, and mental health. The patient also should also be given
chances to connect out to others whenever appropriate so that he can remain
involved participants in their own lives rather than simply waiting for the doorbell
to sound. Individuals often need motivation and direction on how to be
innovative and productive with their time.
