Memorial Medical Center (CMMC), Wilson County, and Wilson County ESD#3 are 
partnering to offer Moderna  COVID‐19 vaccination clinics.   These clinics are for 1st doses only. 
 
Clinic days:  Wednesday, March 31, 2021 
    Thursday, April 1, 2021 
 
Clinic location:  LaVernia Chamber of Commerce Hall

      LaVernia, Texas 78121 
 
You must register for an appointment.  Walk‐ins are not accepted.  Each person expecting to 
be vaccinated must register individually.  Multiple registrations for the same person will be 
cancelled.   
 
WHAT YOU NEED TO KNOW 
 You must be 18 years or older to receive a COVID‐19 vaccine. 
 You must not currently have symptoms of COVID‐19 or have been diagnosed with COVID‐
19 within the past 10 days.  
 You cannot have received any other vaccine within 2 weeks prior to clinic date

and time to the email you provided.  If the time and confirmation # in your email is 
blank, your registration WAS NOT successful and you will have to try again.

o ID  ‐ driver’s license preferred, but not required 
o Appointment confirmation 
 Arrive no more than 15 minutes prior to your appointment.
