Group/Self Evaluation

Directions
Answer each question below honestly and fairly by considering each group member’s contribution, level
of cooperation, and attitude as you worked together to complete your health promotion program.

1. Group Evaluation

 Identify each group member, including yourself, by inserting each person’s name in the
appropriate location in the table below.
 Then assign each person a “grade” out of 100% which reflects their contribution, level of
cooperation, and attitude as you worked together to complete your health promotion program.
 You must give each person a different score.
 Provide a well-stated reason for the grade you give to each person.

Name Grade Reason

(out of 100%)
Abby Christensen 70 With everything that happened this semester, I
wasn’t able to give my all, but I think with the part I
completed and what I was able to improve on and
help with in the latter part of the course deserves
this grade.
Michaela Whitaker 100 She did a good job of initiating ideas and hosting
meetings. She also was a good leader for this
project and had good ideas for the program.
Ksenia Kulakova 99 She was helpful with sharing the screen and
resources during zoom meetings to help us move
forward with our planning. She was also helpful
with coordinating with me.
Cecilia “Pamela” 95 She attended the meetings and gave the needed
input for the different assignments.

2. What did you learn about yourself working with the other group members?

I learned that different events that happen in life will affect those around you. Because of the difficulty
of attending meetings and giving a 100%, it then made it difficult for my group members to carry the
load at times, which I feel bad about. I hope I was able to make a big enough difference with my
improvements later on when helping with this project.
3. What are your feelings about planning a program compared to the first of the semester?

I realized that there are a lot of different parts that go into planning and implementing a program, but
that each step is vital to move forward with your progress. For example, just a simple mind map and an
idea can then take off into research, program structures, findings for funding/resources, networking,
budgeting, applying for grants, and truly understanding your target population, setting, and their needs
in order to have a successful program. While these steps take time and hard work, the positive outcome
these programs can have on the overall health and wellness of the community and public is well worth
it.

4. Rate your confidence level regarding the following statement and explain your answer:

 I feel confident to plan a program from what I have done in this class.
 Use a scale from 1-4 where :
o 1 = Not at all true, 2 = Hardly true, 3 = Moderately true, 4 = Exactly true

Like I mentioned in the previous answer, the breakdown of each step of creating and implementing a
program has given me an idea of everything that is involved with this being created. I feel like I was able
to learn a lot from the readings, power points, and from this project but again with the energy/time I
wasn’t able to give at times, I would rate my confidence as a 3 of planning a program.

5. What was the biggest challenge for you with this project?

My biggest challenge was trying to mentally and literally be able to give 100% of the needed effort for
this course due to the deaths I had in the family and having Covid. While these experiences were hard
and made it difficult to succeed for a while, I feel like I was able to make quite an improvement with my
input and help. I am grateful I was able to have patient and understanding group members.
