Meal Drop-Off Slip

Please print, fill out, and drop off along with meal!
(No printer? Copies can be picked up from the Russell Alliance Church)

INGREDIENTS TO AVOID:
Please take the time to read through the list and check off each one. The side-
effects of these foods can be very serious for Megan. The Norths appreciate
your help, and this checklist will give them (and you!) the peace of mind that
your meal is safe to eat while on chemo and won’t interfere with Megan’s

If along the way you realize that a wayward ingredient has made it into the
dish, just check the last box – OOPS! – and Megan will gladly put the meal in the

PREFERENCES: - Butter, milk, yogurt, and sour cream (must be before Best Before date)
- Ricotta, cream cheese, or cottage cheese (before the BB date)
Small meals (2-4 portions only) are much - Fruits and vegetables, as long as they are fresh and not over-ripe
