INTERNSHIP AGREEMENT

This is an agreement between __________________________________


(intern) and ___________________________________ (Cooperating School). The
purpose of this educational internship is to provide the Intern with an opportunity to
complement his/her formal learning with practical knowledge, skills and desirable
attitudes and to gain hands-on experience.

Conditions of the Agreement:


1. An internship is related to an educational purpose, and the Student Intern is not given
workload intended for a regular teacher. S/he does not replace or displace any teacher of
the Cooperating School.
2. The education received by the Student Intern from the internship is for the benefit of the
Student Intern, which prepares him/her in his/her future teaching job.
3. The Student Intern will receive direct and close supervision by a Cooperating Teacher
duly assigned by the Cooperating School’s Principal/School Head.
4. The learning experiences to be acquired by Student Teacher adapts to the standards set in
the new normal.
5. The Cooperating School does not derive an immediate advantage from the activities
performed by the Intern, such as performing tasks not in line or unrelated with the
internship program.
6. The Cooperating School shall encourage the Student Intern to develop his/her personality
and professionalism, and to the extent as possible, protect him/her from physical or moral
danger and/or violation of R.A. 9208 and R.A. 9262.
7. The Cooperating School shall provide feedback mechanism to the Student Intern who
aimed for the improvement of his/her performance.
8. The Cooperating School shall provide monitoring and evaluation reports or other
information on the student interns’ performance as may be required by the School of
Teacher Education of J.H. Cerilles State College such as a certificate of completion,
accomplished evaluation sheets, and other documents included for purposes of describing
Student Intern’s performance.
9. The Cooperating School notifies and provides at least five days written notice to JHCSC
of a Student Intern’s breach of agreement or misconduct in the internship premises prior
to the Cooperating School’s decision to suspend or terminate the contract.
10. The Cooperating School shall be given flexible incentives as a gesture of gratitude in the
participation of the internship program.
11. The School is not liable for injury sustained or health conditions that may arise for the
Student Intern during the internship.
The Intern agrees explicitly to and acknowledges the following:
1. To follow a regular internship schedule determined by the Cooperating School and
JHCSC;
2. To demonstrate honesty, punctuality, cooperative attitude, and open-mindedness;
3. To practice professionalism in his/her relationship with the teaching community by
observing confidentially with regard to any matters that concern the Cooperating School,
and by being respectful when dealing with his/her Cooperating Teacher, Principal,
College Supervisor, and others;
4. To become acquainted with the existing curriculum and materials in the Cooperating
School;
5. To obey the policies, rules, and regulations of the Cooperating School and comply with
the School’s practices and procedures;
6. To familiarize himself/herself with the culture, structures, and systems of the Cooperating
School;
7. To submit to the Cooperating Teacher required information/documents on the internship,
including related assignments and reports;
8. To make every effort to become involved by participating actively in school-related
activities, however, should prioritize academic-related activities;
9. To meet regularly with the Cooperating Teacher and College Supervisor to discuss
professional concerns, questions, or problems related to the internship;
10. To assist the Cooperating Teacher in various teaching tasks not limited to preparation and
presentation of learning materials to the learners;
11. To develop instructional and assessment materials, and have them checked by the
Cooperating Teacher for correctness and appropriateness;
12. To be receptive to constructive criticisms from the academic community;
13. To strive to make teaching and learning meaningful for the pupils/students by using
appropriate teaching methods and strategies learned in the theory courses;
14. To finish the required 280 hours for internship inclusive of class observation, online
teaching, and consultation with the College Supervisor;
15. To render a minimum of three (3) rated teaching demonstrations in online delivery mode
to include final teaching demonstration in his/her field of specialization;
16. To make regular entries in the Student Teaching Journal and Student Teaching Portfolio
and submit them on time; and
17. To display social etiquette in the connected world.

I have read and understood the contents of this contract and the underlying
requirements in the Internship Program.
