Reynaden Galvan is 19 years old female. She is from Villa San Isidro Lagangilang, Abra. She
was born on February 20, 2001 at Abra Provincial Hospital. She is a Filipino citizen and her primary and
secondary language is Ilocano and Tagalog. She is single, her religion is Roman Catholic. She’s a college
student currently taking up Bachelor of Science in Business Administration.
Reynaden Galvan is suffering from severe stomachache “nagsakit tiyan ko agkaratakkki nak, ti
ammok nga gapo na keta jay nangan nak ti pipino nga adda suka na” as verbalized by the patient.
2 days prior to admission, client experienced severe stomachache because of eating cucumber
with vinegar. She rated her stomachache 8 over 10. She missed her class because she is unable to stand
up.
Client was born normal, without any illness during her childhood. Hasn’t gone any surgery, has
no allergies or accidents.
Client’s father is 55 years old and her mother is 52 years old both have no illness.
