SUBJECT Revised SPMS Forms

DATE November 6, 2017

Pursuant to COA Memorandum no. 2015-008 dated August 7, 2015 on the

Guidelines on the Implementation of the COA SPMS and relative to COA
Unnumbered Memorandum dated September 14, 2017 on the Revised Instructions
on the Preparation and Submission of the Sector/Office/Divisionllndividual
Performance Commitment and Review for Calendar Year (CY) 2017, may we
request that the Individual Development Plan (lOP) of your respective personnel
together with the Summary List of Individual Performance Ratings for CY 2016 and
1 st semester, 2017 be submitted to the Human Resource Management Office
(HRMO) on or before December 15, 2017.

In order to improve team performance and develop individual potentials under

your respective Sector I Office, we highly encourage you to have a coaching session
with your personnel. The Performance Monitoring Form and Coaching Report shall
be accomplished for the purpose.

Attached are the revised SPMS forms which are accessible and downloadable
in the COA website.

Those who have already submitted their documents using the old SPMS forms
are deemed to have complied with the required submission.
