                              Re: Absent during Monday flag ceremony dated April 05, 2021.

           DATE: April 11, 2021

1. Reference: Memorandum Circular 08-001 dated ___________ re: Absent during

Monday Flag Ceremony dated April 05, 2021.

2. In connection with the above reference, please be informed that that about 5:00
am dated April 05, 2021 the undersigned could not attend the said flag ceremony
because of a car accident rendering him to be injured and unable to attend the
flag ceremony.

3. Moreover, please be informed that the undersigned called and informed his
immediate supervisor the moment he was brought to the hospital for the
treatment of his injuries. Hence, the undersigned was unable to attend the flag
ceremony due to the reasons mentioned above and therefore seek your good
office for consideration hereof.
