AFFIDAVIT OF SERVICE

         I, NICANOR D. ANGELO, of legal age, Filipino, single

and a resident of 3333 Washington Street, Pio del Pilar,
Muntinlupa City, after having been duly sworn to in
accordance with law, depose and state:

1. That I am employed as a messenger of Atty. Umberto Dela

Cruz with office address at Muntinlupa City, Metro Manila;

2. That on March 14, 2021, I served a copy of the following

pleading/paper by registered mail in accordance with Sec. 10
of Rule 13 of the Rules of Court:

Petition for Cancellation and Correction of Entry in the Birth

Certificate of Ethan Erasquin

    in Case No. 123456 entitled “Clarisse Erasquin vs. Rafael

Gonzales " by depositing a copy in the post office in a sealed
envelope, plainly addressed to the respondent at his
residence address at; to public respondent Philippine
Statistics Authority and Civil Registrar of Muntinlupa City,
with postage fully paid, as evidenced by Registry Receipts
No. 1111 attached and with instructions to the post master
to return the mail to sender after ten (10) days if
undelivered.
         IN WITNESS WHEREOF, I have hereunto set my
