Task 1: Complete the missing part in the report.

Situation: An engineer who works for FA company writes a report to his boss about the
problem of incorrect packing.

Defect:
The wrong products were sent to the customer. Normally ‘ 2 blocks in 1
rail’ should be sent but ‘1 block in a rail’ was sent.

Investigation:
The problem occurred because the FA inspector is new, and she did not know
about the product code on the order card.

Countermeasure: Change the system from lumom to all tore dance


errors in this process.

Temporary:
The inspector will be informed about the product code and its importance.

Permanent:
Restructing a new employee to receive training before starting work.
Task 2: Practice writing report

Situation: You are an engineer who works in the maintenance department of a machinery
company in Thailand. The KB-II machines are out of order. You consult
Mr. Otaka about the machine’s problem when the problem occurs. You follow
his suggestion regarding how to check and test the machines.

You have to write an investigation and problem-solving report to other relevant


engineers, including your boss, about the KB-II machine. You need to think of the
details of machine’ problem and how to solve the problem yourself. You may
need to use the information below (cause of problem) to help you write the
report, or you can create the cause of the problem yourself.
Cause of problem : clogging in the lubricant pipe

Defect: clogging In the lubricant pipe.


Investigation: problem are rising from workers using lubricants with twice
viscosity of the standard packaging boxes are very similar.
Counter measure: replace the packing with new lubricant for a different color.
Temporary: all the engine oil has been changed and clean immediately.
