1. SAMPLES
The following report details the preparation, testing and analysis of One (01) coal sample from
CV. Bakti Puha Utama. The sample was received at PT. Geoservices Banjarbaru Laboratory
on November 14, 2016. All sampling of the sample was completed by CV. Bakti Putra Utama
or agent. The sample inspection is listed below:

Under instruction from Mr. Hendrie, (Request dated: November 13, 2016) the sample were then
