AFFIDAVIT OF LATE SUBMISSION OF MODULES

I, ELIZABETH N. BONGGO, of legal age, single, Filipino citizen and a resident of Balangay,
Transville, Poblacion, Quezon, Bukidnon, Philippines, after having been duly sworn to in accordance
with law depose and say:

1. That I am a student of Philippine Countryville College, Inc. taking up Associate in Computer

Technology;

2. That during the first semester of school year 2020-2021, I was not able to submit my
requirements in ITCC 5 – Information Management due to localized lockdown ;

3. That the Office of the Municipal Mayor of Quezon, Bukidnon issued Executive Order No. 35,
an order declaring sections of Balangay, Poblacion, Quezon, Bukidnon under localized
lockdown;

4. That movement (incoming/outgoing) of all persons within the area is strictly restricted;

5. That failure to abide by the said Executive Order shall be meted with appropriate action in
accordance with relevant laws and regulations;

That this Affidavit is executed by me for the purpose of attesting the true facts above stated and
to request Sir Jonathan C. Villas, ITPE 1 – Object Oriented Programming instructor, to accept late
submission of my requirements in his subject.

IN WITNESS HEREOF, I have hereunto set my hand this 4th day of April , 2021 at
