We would like to suggest you contact our Customer Care Unit to add Kyle Sproul as a secondary owner for your pet’s policy. As
this information has not been listed on the policy it may cause unnecessary delays with future claim submissions. If you would
like to add a secondary owner on this policy, please contact our Customer Care Unit and they will be happy to assist you.

in writing with Our Reassessment Request Form and include new or additional medical information supporting Your claim.
