CERTIFICATION

This is to certify that the transportation expenses being requested by Mr. Alano R.
Antalan for the month of April & May 4-5, 2021 relatives to the conduct of
Inspections/ repair and preventive maintenance of DSWD Building Facilities and
other DSWD related transactions at Record’s Bldg., Bauang, La Union; Ilocos Sur
and Ilocos Norte from DSWD FO-1, CSF, La Union (Official Station) to HFG-Agoo, La
Union(destinations) back to FO1; from DSWD FO-1, CSF, La Union (Official Station) to
Ilocos Norte- Ilocos Sur,(destinations) back to FO1& other areas at the City of San
Fernando, La Union in the total amount of Php3,090.00 (back & forth) is for
reimbursement of equivalent cost in lieu of hiring public vehicle.

This further certify that the reimbursement of personal use of vehicle is limited to the
round trip cost of ordinary mode of transportation.
