This application form is used to notify the Department of Infrastructure that the vehicle has been scrapped and should not
be used for any other purpose.

If the vehicle is an Insurance write off please use form MVR8

If you wish to retain the registration number then use a C1 form or MVR2 form to move the number onto another vehicle.

If the vehicle has a valid licence disc then this can be submitted using an R1 form for a refund of any full months
remaining at the time of application.
