Middle Rock Quarry Road, Baguio City, Philippines after having been duly sworn to in
accordance with law, depose and say that:

1. I have been employed as a Staff Nurse for more than three (3) years now at .

2. I hereby declare that during my employment with Baguio General Hospital and
Medical Center, I have never been charged nor convicted of any crime or
criminal offense, nor be the subject of any criminal investigation nor
prosecution;

3. This affidavit is executed in support of my request for clearance and my being

granted of all benefits I may have earned and may claim from Baguio General
Hospital and Medical Center, to lend credence to the facts above-stated, for all
other legal intents and purposes it may serve best.
