AFFIDAVIT OF ACKNOWLEDGEMENT OF PATERNITY

I, MICHAEL CASTILLO BALE, Filipino, of legal age, and a resident

of Tago, Surigao del Sur, after having been duly sworn to in accordance
with law, depose and state:

1. That I am the biological father of KEITH BRYALLE BOTRON,

who was born outside of wedlock on April 24, 2021 at Tago, Surigao del
Sur;

2. That at the time of birth of said child, I was not married to his
mother, JESSABEL NORONIO BOTRON;

3. That I hereby acknowledge my paternity/affiliation to the said

child;

4. That consequently, I hereby give my consent for my child,

KEITH BRYALLE BOTRON, to use my last name in his Certificate of Live
Birth and be known as KEITH BRYALLE BOTRON BALE;

5. That I am executing this Affidavit to attest the foregoing facts

and in connection with my desire to register the birth of my child under my
surname with the Local Civil Registrar of Tago, Surigao del Sur and for
whatever legal purposes this may serve.
