Directions: Each student is required to participate in a minimum of 20-60 minutes of physical activity/weight training at
least 4 days of the week (Monday through Sunday). If you train more than 4 days, awesome, write it down, but 4 days
is the requirement. Complete all columns of the log below. There are a bunch of examples of activities listed on info
sheet, and plenty posted on Schoology. Workouts will be posted on your Schoology Weight Training Course Page.
Before you submit, be sure a parent or guardian signs electronically.
