YSR Kadapa . my ZPPF Account number is 21723 .

My wife smt. S.visalakshi aged 49 years is suffering from illness and the doctor said that nearly an
amount of Rs 2 50, 000 ( Two lacks fifty thousand Rupees only ) . will be needed for her
treatment . So Please sanction the amount of Rs 2 50, 000 ( Two lacks fifty thousand Rupees
only ) from my ZPPF account towards Non-Refundable loan as early as possible.

Along with my representation, I enclose my ZPPF Non-Refundable loan application form , Doctor
certificate , bank pass book first page Xerox copy and Xerox copy of SR 1 st and 4th pages.
