Scoring You will need to keep track of your answers for each dimension. Give yourself
1 point for ‘Never,’
2 points for ‘Sometimes,’
3 points for ‘Often,’
4 four points for ‘Most of the time,’
5 points for ‘All the time.’
The higher your score, the higher your level of wellness is for that dimension.
