of MYSTIQUE WHITE ENTERPRISE, do hereby name, constitute and appoint ALEXANDER DUMAGAT, of
legal age, Filipino citizen and a resident of Quezon City, to be my true and awful attorney, for me and in
my name. pace and stead, to do and perform the following acts and things to wit:

1. To represent me and in my behalf in the processing of my business closure in the Bureau of

Internal Revenue, RDO – 034;
2. To receive any and all documents related to the closure of business from the concerned
government agencies/offices;
3. To pay any and all such expenses that may be necessary to effect the closure of the said
business;
4. To sign and execute the necessary documents to carry into full effect the power herein
conferred and to do any other related and/or incidental task that may be needed to fully and
effectively carry out the authority herein conferred upon him;

HEREBY GRANTING unto our representative full power and authority to execute and perform every act
