Notes:
1-This medical examination will be valid for three (3) months and can be extended for 1 month after OQ OH approval.

2-The tests indicated above shall be recorded in the “Medical Evaluation Report” as per OQ template and the report will be assessed by OQ OH Medical Clinic for approval, in case of
any abnormal results, a complementary assessment by a specialist will be requested to conclude the assessment.

3-ECG report shall present 2.5 second tracing of each of the twelve leads and interpretation. The results shall be evaluated by a cardiologist who will interpret the results and issue
the conclusive report
4-The laboratory report shall present results of the blood and reference range. The results shall be evaluated by a clinical pathologist who will interpret the results and issue the
conclusive report.
5-If the FBS indicates abnormal results, the CONTRACTOR shall perform HBA1C test.
Critical Activities Definition

Working at height
Applies to the gaining of access to work areas and the performance of activities where there is the possibility of people falling
1.8 m or more. In cases of specific legislation stipulating a smaller distance, this will prevail.

Working in confined space


Applies to all activities involving access to and performance of work in spaces defined as confined.

Working with electricity


Applies to activities in electrical facilities and electricity-related services in the areas of generation, transmission, distribution
and consumption, including the stages of planning, construction, assembly, commissioning, operation, maintenance, return to
service, emergencies, modifications to equipment and decommissioning.

Working near rotating machinery


Applies to all machinery, equipment and operational systems that possess moving parts or that involve materials or fragments
of materials being projected into the air or falling.

Mobile machinery operation


Applies to all mobile machinery, such as: excavators, loaders, wheeled or crawler tractors, motor graders, motor scrapers,
backhoes, off-highway trucks and other trucks, whether owned, leased or rented by the contractor.

Heavy lifting operation


Applies to all activities involving hoisting, moving or transporting cargo using equipment such as mobile cranes, vehicle-
mounted articulated cranes, cargo elevators, tower cranes, overhead cranes, electric hoists, ore handling equipment,
monorails, gantry cranes, cargo handlers and tire handlers.

Driving vehicle
Applies to all vehicles able to hold up to five passengers, cargo vehicles, minivans, vans, microbuses, buses and automobiles
able to hold up to 12 passengers, whether owned, leased or rented by the contractor.

Working in noise area


Applies to workplace with noise sources at or above the exposure limit (85 dB(A)).

Working in extreme heat


Applies to all work conducted in high temperature conditions.

Exposure to chemicals
Applies to all activities involving the handling, internal transportation and storage of hazardous chemical products in any
physical state (solid, liquid or gas).

Pulling, pushing or carrying weight


Applies to all manual handling involves any transporting or supporting of any load by one or more workers, and includes
lifting, putting down, pushing, pulling, carrying or moving a load.
