Preparing an Online Evaluation Tool
Performance Objective:

Using the provided course evaluation instrument, prepare a course evaluation tool using Google Forms.

Supplies:
LMS, evaluation form

Equipment:
Computer, internet connection

Steps/ Procedures:

1. Go over the instrument. As you go over the instrument, reflect on the elements and take note of the questions
that you think are critical for evaluating eLearning sessions.
2. Proceed to your Google browser and look for Google Forms in the menu on the upper left corner. Select a
Blank Form and input the questions you took note of in item No.1.
3. Once done, click Send and select the <> icon.
4. Copy the link/ HTML code generated.
5. Go back to your development site (https://tesdabicol5.gnomio.com/?redirect=0 ).
6. Turn editing on and add a Page.
7. Type Course Evaluation Survey as the name of the page and provide a brief instruction or description.
8. In the Content Box, click the icon and select the HTML (</>) icon afterward.
9. Paste the embed code in the content area and click the HTML (</>) icon again.
10. Configure other settings, save and check.
