Incident Type: Report on Damaged PC Speaker, Loss PC Camera and Destroyed Biometric Device

INCIDENT DESCRIPTION

According to Ms. Jonna Mae R. Tagyamon, it was on the 10 th of February when she brought the PC speaker and a pair of
PC camera to school. As soon as Ms. Tagyamon arrived at school, she opened the blue plastic where the mentioned
items were placed. She showed the devices to Mrs. Eva B. Arander, Mrs. Rea O. Torres and Mr. Elmer B. Indoy. Ms.
Tagyamon and Mr. Indoy then tested the speakers and as expected it worked. After testing, the two (2) speakers were
put back into place together with the two (2) cameras. Ms. Tagyamon said that the devices--specifically the speakers that
were tested--were returned back to the box and were safe inside the blue plastic bag.

Ms. Tagyamon together with her colleague Mr. Kerwin Aguilar decided to assemble the two sets of (PCs) Personal
Computers on the 15TH of February at 9:30 AM, they found out that one of the speaker’s wire has been cut intentionally
and one PC speaker went missing.

On the 22nd of February, as Ms. Tagyamon and Mr. Aguilar arrived at school, they again discovered that the speaker which
was mentioned to have been damaged (with its wires intentionally cut) was targeted again and was attempted to be
opened. The wires were taken off of the speaker and was considered missing as well. Mr. Aguilar decided to fix the
damaged speaker by connecting a new wire to make it still functional. Of the same date, the school’s biometric device has
been found destroyed as Ms. Tagyamon saw that the fingerprint scanner’s cover has been forcibly pulled out and the said
cover was nowhere to be found.

March 22, 2021; according to Ms. Tagyamon, she was the first to arrive at school and she noticed that the door of the
teachers’ quarter had already been opened. She then found out the two working students: Ailyn Librando and Lerma
Tano were earlier to set foot on the room than her. At that time, she caught sight of the biometric device and wondered
why it was removed, thinking that perhaps it was detached from the wall after the MOOE Validation. She went straight to
the house of the utility/watchman which was just fronting the school buildling. She handed an honorarium for the month
of February to the watchman who is Mr. Fernando Santiago and she inserted to ask the watchman to check whether he
saw where the biometric device was placed. And as stated by the watchman, the time after the last teachers departed
from school, he went to the quarter and saw the biometric device being laid on Mrs. Miriam G. Reyes bed. He stated that
the wires were cut and he thought that maybe the teachers were the ones who were trying to fix the device.
