Expanded Tertiary Education Equivalency and Accreditation Program (ETEEAP)

Qualifications:
1. Must be a Filipino Citizen, HS graduate or College Level,
2. Has at least 5 years of work experience related to the degree he/she is applying for.
3. Age 25 years old or above, and
4. Able to show/submit certificates of trainings and seminars attended.

Enrollment Requirements:
1. Accomplished ETEEAP Application Form with 1x1 picture,
2. Certificate of employment,
3. Detailed job description signed by the Human Resource Department
          In-charge,
4. Comprehensive Resume/Curriculum Vitae
5. Certificate of seminars/trainings attended or conducted,
6. Pictures at work with description,
7. PSA Authenticated Birth certificate,
8. Official Transcript of Records (for non-USJ-R) and other transfer
credentials/certificate of deficiency (for USJ-R students)
High School Card (evidence of graduation for high school graduates)
9. Transfer Credential or Honorable Dismissal for transferees,
10. NBI Clearance/Barangay Certificate,
11. Marriage Certificate (for married female)
12.Community Tax Certificate/Cedula (to be written at the last portion of application
form), and
13.Good moral Certificate
14. Colored 2x2 picture.
15. Others

NOTE: Applicant must submit three (3) folders: One (1) original copy to be returned
after the validation assessment, and one scanned copy (1) for the University file.

 2 long, paper folders
 1 long clear book
