STEP 1: PLEASE ACCOMPLISH THE REQUIREMENTS: Kailangan po ang isang applicant ay 25 YEARS OLD ABOVE
at may 5 YEARS ABOVE na working experience na related po sa course na plano ninyong kunin under the ETEEAP
Program. CURRENTLY EMPLOYED po dapat ang isang applicant OR MAY NEGOSYO PO NA MAY LEGIT PERMITS.
Ang tangapan po ng application at Requirements ay mula po:
LIST OF REQUIREMENTS:
Part I – Personal
1. Accomplished ETEEAP Application with Original 1x1 picture. (Please print it in LONG BOND
PAPER) (Original) Please fill up the form with a complete handwritten only (Optional)
2. Letter of Introduction addressed to Dr. Benilda N. Villenas – Director, ETEEAP
3. Resume (w/ Original 2x2 picture). Please include Job Description from your current work and
previous working experience (Original)
4. NBI OR Police Clearance (Original)
5. Barangay Clearance (Original)
6. PSA Birth Certificate.( Photocopy) and Marriage Contract (PSA) (For Female Applicant Only).
7. Photocopy of Transcript of Records (TOR) and Photocopy Diploma (for College graduate) and
Photocopy Transcript of Records (TOR) only (for College Undergraduate).
8. Photocopy Transfer of Credentials and Photocopy Certificate of Good Moral Character. (For
College Undergraduate and Graduate- Photocopy). For High School Graduate Photocopy F137
and Certificate of Good Moral Character.
9. Cedula (Community Tax Certificate) for last part of application form. (Original)
10. Letter of recommendation from current employer allowing you to study ETEEAP Program at
Manuel S. Enverga University Foundation. (Original)
11. For OFW photocopy of Passport., For Local Applicant – Photocopy of any PRC License (If
Applicable)
12. Print out (Google Map) – Location of Your company for worksite visit.
STEP 2: Ilagay ang lahat ng requirements sa 1 long white folder na naka-paper fastener (1 set only). Sunod-sunod po
dapat according po sa list of requirements. Dapat po complete na po ang requirements
* Para po sa Local Applicants kung ang kulang na lang po ninyo ay Transfer of Credential/Honorable Dismissal
and Certificate of Good Moral Character or F137 for High School Graduate from last school attended. Puwede na
po kayo magpasa ng requirements. Pakidala na lang po ang mga nasabing dokumento sa araw ng schedule ng
interview.
* PARA SA OFW APPLICANTS Dapat po 1 month before kayo bumalik sa Pilipinas matangap ko na po ang
documents thru LBC. Mag messege po sa fb page na “ENVERGA ETEEAP CENTER” Kung kelan po dating ninyo
sa pilipinas para maline-up sa schedule ng interview at pakiindicate kung gaano katagal lang ang inyong bakasyon
sa plilipinas. required po ang isang ofw applicant na pumunta sa university para po sa 1 day for interview at
another 1 day for entrance examination (pag nakapasa po sa interview). FOR TOTAL OF 2 DAYS. Puwede na po
magpa-interview Mula po November 05, 2019 Until – May 28, 2020.
Note Ang NBI, OR POLICE CLEARANCE AND BRGY. CLEARANCE, Cedula, Transfer of Credentials and Certificate
of Good Moral Character and F137 for High School Graduate). Pakisubmit na lamang po ang mga nasabing
documents sa mismong araw po ng interview. Kasama po ang original transfer credential/honorable dismissal and
Certificate of good moral character at para po sa High school graduate Original F137 and Certificate of good moral
character
STEP 3:
Option 1 (For Local Applicants) Pumunta po ng personal sa ETEEAP Office (Alumni Building) dalhin po ang inyong
application at requirements. Magbayad po ng application fee na Php 1,500.00. Monday to Friday 08:00 am to 03:00 pm.
Magtext muna po kayo 2 days before kayo pumunta kung meron pong pasok o wala.
Option 2: (For Local Applicants) Ipadala sa LBC ang documents ninyo kasama po ang ang original bills payment slip
and original receipt from union bank see below for sample of form:
(For Ofw Applicants) Dito na po kayo magbabayad ng Application Fee (Php 1,500.00) sa Enverga University
pag may schedule na po kayo ng interview. (Monday to Friday)
At ipadala sa address na ito ang inyong mga documents: (Local and OFW Applicants)
STEP 4: Intayin ang text mula sa ETEEAP Center para sa schedule ng interview at dalhin ang lahat ng original documents
na photocopy ng inyong isinubmit. Para sa OFW pakipadala din po ang original passport.
