son / daughter to attend the school commencing with effect from10/01/2021 amid
COVID-19.

Further I confirm that my child does not have any illnesses/ symptoms of covid 19.

I Under take that:

1. I ensure that my daughter /son shall follow all the guidelines and maintain
the complete social distancing and hygiene to ensure safety and health.
2. I shall provide a face mask and hand sanitizer to my daughter / son required
as per the guidelines of the government.
