APPENDIX D
PET POLICY

On the Effective Date, you entered into a Residential Living Agreement (“Agreement”) with Sabra IL
Operations LLC , to which this Appendix D (or, “Addendum”) is attached. Terms defined in the Agreement
have the same meaning in this Addendum as in the Agreement.

You desire to keep a pet (which does not include a guide or service dog) in the Unit and we consent on the
following terms and conditions:

1. Pets are not allowed in areas of the Community where food is prepared or being served.
2. Pets are allowed only in the Unit, unless the pet is being taken to and from the outside.
3. Pets being taken to and from the outside must be on a leash and under your control at all times.
4. Only one pet shall be allowed (or a number determined reasonable by us), and it shall be only a dog, a
cat, a bird, or fish.
5. You shall maintain, and provide evidence to us, of current and proper immunizations of your pet, in
accordance with the laws, regulations and health customs of the city and county in which the
Community is located.
6. You shall be responsible for your pet’s personal needs such as exercise, feeding and collection/disposal
of pet waste in and about the Unit and on Community property. If you become unable to care for your
pet’s personal needs, as determined by us in our sole discretion, it is your responsibility to locate
alternate housing/accommodations for your pet.
7. You shall prevent your pet from causing any disturbance which would constitute a breach of the
covenant of quiet enjoyment of others and, when your pet is not within the Unit, it will be at all times
controlled (via physical restraint such as a leash, animal carrier, crate, etc.) and under your immediate
supervision and responsibility.
8. You shall be held liable for injury to others and damage to the Community, including any common
areas, caused by your pet.
9. A breach of these conditions shall allow us to terminate this Addendum or the Agreement, at the
Community’s option.

The information above constitutes the full extent of this Addendum.
