I, PROTOCENDO C. RAGO, of legal age, married, Filipino Citizen
and with address at Sta. Cruz Barangay, Isabela City, Basilan after having been duly sworn to in accordance with law, do hereby depose and say:
1. That I am the owner of Lot 4-C-11 of the subdivision plan Psd-09-
012264 situated in the Barangay of Eastside, Isabela City, Basilan covered by Transfer Certificate of Title No. T-9277 which is attached herewith as Annex “A”.
2. That I am likewise the owner of Lot 3-B-2-C-5-D-2 consisting of one
hundred forty-six square meters (146 sq. m.) situated also in Barangay of Eastside, Isabela City, Basilan as evidenced by a Deed of Sale which is attached herewith as Annex “B”.
3. That Lot 4-C-11 described under paragraph one is adjacent to Lot 3-
B-2-C-5-D-2 described in the next preceding paragraph.
4. That currently I am applying for the approval of subdivision plan
involving Lot 3-B-2-C-5-D-2 with the Land Sector of the Department of Environment and Natural Resources Office. That as a condition for the approval of the said subdivision plan; I have to provide a road right of way to Lot 3-B-2-C-5-D-2.
The Sketch Plan of Lot 3-b-2-c-5-d-2 is herewith attached as Annex
“C”. 5. That, I hereby undertake to provide a road right of way to Lot 3-B-2- C-5-D-2 through Lot 4-C-11 three (3) meters wide which shall serve as the access of the former Lot to the city road.
