PETITION FOR DOMESTIC ADOPTION OF MINOR

The petitioners, through the undersigned attorney, and unto this

Honorable Court, respectfully avers that:

1. The petitioners KAI COLOCAR and DEEANA COLOCAR are

husband and wife, thirty (30) and thirty-two (32) years of age,
respectively, Filipinos and residents 10th Floor One Spatial, R. Mapa St.,
Tabucan, Mandurriao, Iloilo City, Philippines.

2. They have no legitimate children or descendants and hereby

desire to jointly adopt a minor, named Haechan Gilmore, seven (7) years
of age, legitimate child of Rory Gilmore and Dean Luceo.

3. The mother of said minor is now dead, and only his father,
Dean Luceo, is alive.

4. The said father, Dean Luceo, is not insane, nor intemperate

and has execued a Deed of Voluntary Commitment, as evidence by
Annex “A”, hereto attached and made an integral part of this petition.

5. The Department of Social Welfare and Development has

issued a Certified of Availability for Adoption certifying that the minor,
Haechan Gilmore, is available for adoption, as evidenced by Annex “B”,
hereto attached and made and integral part of this petition.

6. The petitioners are qualified to adopt the said minor and

have complied with the requirements udner the Domestic Adoption Act
of 1998 (R.A. No. 8552), and will be able to financially and morally bring
up and edicate said minor proprerly and adequately.

WHEREFORE, it is respectfully prayed, that upon due notice and

hearing, judgement be entered adjuding that the minor child, Haechan
Hyuck, be declared to all legal intents and purposes, the child of the
herein petitioners KAI COLOCAR and DEEANA COLOCAR, and that his
surname be changed to that of the petitioners.
